چکیده

There is a need for evidence-based usability principles to support the design of usable medication-related computerized CDS functions and systems. Such evidence requires establishing scientific relationships between usability principles, their violation in terms of usability flaws, issuing usage problems and their consequences or outcomes in the clinical work and patient care. This kind of evidence is not currently directly available in scientific evaluation studies of medication CDS functions. A possible proxy to seek evidence is systematic review of existing scientific evaluation reports. We rely on a four-stage framework describing the chain of consequences and inferences linking usability principles to clinical outcomes to design the systematic review methodology and interpretation principles. This paper describes the four-stage framework and the resulting consequences for the systematic review design.

Design of decision support interventions for medication prescribing

OBJECTIVE Describe optimal design attributes of clinical decision support (CDS) interventions for medication prescribing, emphasizing perceptual, cognitive and functional characteristics that improve human-computer interaction (HCI) and patient safety. METHODS Findings from published reports on success, failures and lessons learned during implementation of CDS systems were reviewed and interp...

Bates DW. Prospective evaluation of medication-related clinical decision support over-rides in the intensive care unit. BMJ Quality and Safety 2018, ePub ahead of print

Introduction Clinical decision support (CDS) displayed in electronic health records (EHRs) has been found to reduce the incidence of medication errors and adverse drug events (ADEs). Recent data suggested that medication-related CDS alerts were frequently overridden, often inappropriately. Patients in the intensive care unit (ICU) are at an increased risk of ADEs; however, limited data exist on...

A clinical decision support needs assessment of community-based physicians

OBJECTIVE To conduct a grounded needs assessment to elicit community-based physicians' current views on clinical decision support (CDS) and its desired capabilities that may assist future CDS design and development for community-based practices. MATERIALS AND METHODS To gain insight into community-based physicians' goals, environments, tasks, and desired support tools, we used a human-compute...

Review Paper: Medication-related Clinical Decision Support in Computerized Provider Order Entry Systems: A Review

While medications can improve patients' health, the process of prescribing them is complex and error prone, and medication errors cause many preventable injuries. Computer provider order entry (CPOE) with clinical decision support (CDS), can improve patient safety and lower medication-related costs. To realize the medication-related benefits of CDS within CPOE, one must overcome significant cha...

Towards meaningful medication-related clinical decision support: recommendations for an initial implementation.

SUMMARY Clinical decision support (CDS) can improve safety, quality, and cost-effectiveness of patient care, especially when implemented in computerized provider order entry (CPOE) applications. Medication-related decision support logic forms a large component of the CDS logic in any CPOE system. However, organizations wishing to implement CDS must either purchase the computable clinical conten...
